All Clubs are required to submit the following forms for Membership to Karate Manitoba:

Club Registration Form » One form per club, to be completed by the Head Instructor.

Membership Form » Note: one registration form PER STUDENT to be submitted. Students that do not submit a membership form are NOT covered by Karate Manitoba. All members must register individually through their dojo.

NOTE:  To take part in any Karate Manitoba tournaments, High Performance Training Program, NCCP and Karate Canada Officiating Program trainings, you must be a full club member in good standing.
